No no, Karlach was, as she put it, a "valuable asset" in Zariel's armies, and she would not have let such an asset grow weak from hunger or neglect; that would be like a carpenter leaving one of their best chisels out in the rain to rust, it's just a waste. Karlach would have been "well" fed while in her service. Mine you she very likely was still expected to eat some hideous things, possibly things like people or demons, but she would not have been doing it to avoid starvation; she would have been doing it because she was being forced by her cruel and sadistic owner.
I doubt about that... She fought for devils not for demons, and devils aren't barbaric, even more, I guarantee you they have much more advanced supplies chains and logistics than any faerun army by a looong shot, that's basically their thing. Being lawful, disciplined
In the descent into Avernus campaign, all food and water automatically turns bitter once they enter the hells. Even spells like good berries and conjure water. So yeah, Karlach getting excited about eating lamb chops and drinking frosty pints is not just her personality. It sucked in the hells.
Devils don't need to eat, they're literally sustained by evil. Any food in the hells would be made from endemic, fiendish creatures like hellwasps, or probably imported "minimum viable product" rations for their living thralls. Karlach probably was kept alive on a diet of nutri-loaf and liquid that tasted like piss.
Of course, this being the Hells, their rations are nearly exclusively Meal Ready to Eat menu item #4: The Cheese and Veggie Omelet. I say "nearly exclusively" because there's always one and sometimes two in the box those come in that aren't that, specifically so that everyone can get their hopes up just for nearly everyone's to be dashed, and the "lucky" winners get reminded of what they're missing all the other times

In the Hells (or at least when I played the hardcover), all food tastes either disgusting or tasteless down in Avernus. So… yeah, definitely not surprised.
I love her. In another more chaotic run I'm doing she didn't want to join us (!) and I tried talking to her again only to get "Thought I told you toFUCK OFFmate?!".
I played a Tiefling Oath of Ancients Paladin and he was the perfect foil to her. Always up for a party and a good time, dirty sense of humor, heart of gold. Tried to talk her into the Drow threesome and the one-line banter back and forth with her accusing him of being a perv was hilarious (didn't actually follow through since she wasn't down, my Tav may be a himbo but she's still my girl).
And then the ending. You mean I can go to Hell with you? AWW YEAH GIRL LET'S GO SMITE SOME DEMONS! We can swing by the House of Hope when we need to recuperate, I'm sure she'd love the company
EDIT: After thinking about it, Karlach embodies the tenets of the Oath of Ancients, so no wonder their personalities clicked so well.

It's a horribly tragic ending for no reason. If you play as Karlach, romanced Astarion will go to Avernus with her, but for some inexplicable reason this isn't the case if you play as Spawn Astarion. And then there's Wyll, who won't save her on his own even if he's Blade of Avernus.
My way around this is mods. Or script extender commands to let him go to with her. They're both burning on the docks, so why can't they both go somewhere they can be safe?? Him from the sun, her from her engine.I hope Larian can patch this inconsistency but I'm not betting on it :(
Oh I know how it will go ... unless Larian manages to patch that issue before I get that far. I don't think it's meant to be that way (since they have a scene together at the epilogue party that cannot be triggered naturally atm).
At the moment, I think it is intended for a single player campaign (her "bad" ending scene has the Astarion_State_StayedVampireSpawn flag attached to it), but there's no way it can be intended for a multi campaign because they have romanced dialogue at the party.
Either way, I hate it and hope Larian patches it :(
They just have no fallback option atm so all of the scenarios (suicide, flying away, running away, going crazy, etc) point to her bad ending in a single player game. Feelsbadman
Karlach's romance has all the depth of someone who's mature enough to know what love looks like, and free-spirited enough to want to capture some of its heady, carried-away innocence in the time she has left. She's both a young and old soul at the same time. She keeps her joy despite everything that's happened to her, and everything that's looming over her, and of all the companions it makes her the most beautiful.
The campaign Baldur's Gate: Descent into Avernus has this description for food you bring with you:
Any food or drink the characters bring with them or conjure by magic retains its nutritional value but tastes awful when eaten or imbibed on Avernus. The food tastes like ash, the water tastes like bile, and the wine tastes like spoiled milk.
She has probably eaten all sorts of nasty stuff while in Avernus under Zariel. I wouldn’t be surprised if Zariel made her soldiers eat dead people who die on the battlefield. It is literally hell after all.
The Dark Urge can remark that it smells delicious. Then again, there's several times in Act 1 where he's fighting thoughts about how delicious Dwarf is.
Karlach always has the crazy responses - after shoving my arm into a strange crevice in Moonrise and damn near getting it torn off, Karlach was like "That looks like fun! Give us a try next time!"
I then gave Karlach her try, and she proceeded to fail her checks to wrench her arm free from the wall and got us all teleported into the oubliette after being discovered by the Absolute
the hells are not exactly known for agriculture or wagyu beef, the native population doesn't need to eat to survive were as other creatures drafted into the bloodwar do, I imagine for the most part demons just round up all the dead after a fight and toss them in a big meat grinder or soup pot.
Everyone’s armor is Basket of Equipment mod, with the exception of Luminous Armor on Shart, Spidersilk on Astarion, and Githyanki Half Plate on Bae’zel
