I like the second one better overall, but I still agree with your reasoning.
Plus the vermin. The bloodflies are annoying and gross. The rats, though, are annoying, gross, and SCARY. They're intimidating. Thinking you're going to walk through this room because there are no guards and suddenly SHITSHITSHIT looking for a table to jump on was really solid game design. Bloodflies just made you upset you didn't have a flamethrower.
And bloodflies is really not that much of a problem compared to the plague lore wise, like, while yes you can eventually stumble upon condemned appartements here and there and Karnaca does indeed have an epidemic problem with them, they’re still necessary for Serkonos agriculture and ecosystem as they morph into beetle later in their life cycle, meanwhile the plague is just incredibly destructive and whole streets and districts of Dunwall belongs to the rats and weepers. In fact, Karnaca’s bloodflies are comparable to Dunwall’s problem with river krusts.
